Acronym for "Get Out of my Emergency Room," from the book "The House of God" by Samuel Shem. In the book, doctors are frustrated by having lots of sick old people turning up in their "ER" (American for A&E). The doctors find the old people difficult to treat, they are portrayed stereotypically as being demented, deaf, immobile and probably beyond the help of medicine. The doctors become tragically desensitised to the plight of their patients, and invent the term "Gomer" as a coded insult.
